Dr Robert Troell from California recently published his study comparing the different types of liposuction he has performed in his practice with interesting results. This study carries significant credibility because Dr Troell has been a national trainer in both Vaser and Body-Jet techniques in the USA and so offers an unbiased view.

In this 150 patient study, Dr Troell compared traditional liposuction (Suction-assisted-liposuction) to Ultrasound-assisted-liposuction (VASER) to Water-assisted-lipouction (Bodyjet). He produced  this study to help both physicians and patients in choosing the right treatment.

Here is a summary of the results:

• VASER resulted in greatest amount of fat removal and highest patient satisfaction
• Bodyjet required about twice as much fluid as two other techniques
• VASER and Bodyjet use less traumatic methods to remove fat than simple suction power

Who can have VASER 4D Sculpt?

The VASER 4D Sculpt procedure is meant for people who are already in good physical shape but have trouble achieving a defined, athletic physique. Both men and women can be treated. VASER 4D Sculpt is not meant for people who are very overweight or obese. The procedure depends heavily on skin retraction to help define the anatomical results. For people who are overly heavy, the skin has stretched beyond where VASER could help it retract. Please ask Dr Jain what other cosmetic options you have if this describes you.

Where can I have VASER 4D Sculpt done?

Honestly, just about anywhere. Men generally have their upper arms, and front & back of torso treated to give the most natural looking results. Women tend to have the same as well as their thighs and buttocks.

How does VASER work?

VASER is an advanced medical ultrasound technology (VASER stands for Vibration Amplification of Sound Energy at Resonance). During surgery, the VASER probe emits sound energy that literally melts fat tissue before it is gently suctioned away. In traditional liposuction, the fat is just sucked away—taking blood vessels, nerves, and important connective tissues with it—resulting in severe pain, a long recovery, and mediocre results. With VASER, the melted fat is easier to suction away, far gentler on the body, and can be sculpted to enhance muscle appearance.

How long does it take?

The larger the area being treated the more time it requires. Treating an area as small as the jowls, for example, would require approximately one hour to treat. A larger area such as the abdomen can take up to two hours. A full-body VASER 4D Sculpt treatment can take anywhere from two to four hours. Dr Jain will give you closer time approximation during your consultation.

Will it hurt?

While every person has a different threshold for pain, we can guarantee you won’t feel anything during the procedure as local or general anaesthesias  are always utilised for your comfort. The procedure uses very small incisions to do the work, so there is little external healing required.  Recovery is quick and relatively comfortable. You need to be realistic in that you will need to be at home for about 1 week post surgery.

Is it safe?

Absolutely! In fact, VASER liposculpture is safer than any other liposuction technique. The technology is designed to target only fat cells. There is no risk of burns or other internal areas being harmed.

How quickly will I see results?

Liposculpture removes fat immediately from the body, so some patients report seeing results immediately after surgery. It is likely that you will see results quickly, with the final results present 3 to 6 months after surgery.

How long will it last?

Once you have fat removed, it is gone permanently. Your new shape is easy to maintain with a healthy diet and exercise regimen. In the event that you do gain weight, your body will retain the proportions that Dr Jain sculpted; for example, a more shapely waistline will stay in the same general shape, with fat developing in better proportions than before. So even if you gain weight later, you will still look better than you did before surgery.

What aftercare will I need after Vaser 4D Sculpt?

You will need about 10 aftercare sessions following your surgery in order to maximise your results and speed up your recovery. Dr Jain will NOT perform surgery unless you agree to commit to this vital part of your treatment package. At Riverbanks, we use the CARE protocol as devised by Dr Alfredo Hoyos. Our therapists have been trained in Bogota, Colombia at Dr Hoyos’ clinic and so have received the most advanced training for your comfort.

Why do I need aftercare?

Your surgery will cause inflammation as natural healing response and as a result swelling will occur. Without aftercare, the swelling can be painful and also potentially complicate the shape created for you during your surgery. It is compulsory for all Vaser 4D and Hi Def patients to receive aftercare as instructed by your surgeon. Therefore we advise all patients to stay locally for at least 1 week post surgery.

3. VASER Hi-Def is Not Dynamic Definition.

VASER Hi-Def results often appear stiff and do not move fluidly with the rest of the body. This is partially because of both the reasons stated in #1 and #2, because VASER Hi-Def does not plan according to each body type or according to where the muscles will move in the future. Dynamic definition encompasses the ideals of 4D Sculpt™, in that once the muscles are defined, they do not appear as though they’ve been carved out of plastic.

Dietary Recommendations following your Vaser surgery

Vaser liposelection will have removed a significant amount of your fat cells from your body especially from those stubborn areas. However, it is important to realise that you do need to look after your new body in order to maximise the results of your surgery.

Many of our patients have commented that they have found it easier to lose weight following VASER than before. As a result their motivation to diet and exercise is maintained for longer and so they achieve better results.

Some people are very resistant to weight loss if they eat starchy food such as breads, rices, pastas, potatoes, milk, chocolates & sweets.

Following my own surgery I adopted a low GI diet together with some other lifestyle changes which led to dramatic weight loss and also improved physique.

DIET

For the next 3 weeks, try to avoid the following ingredients for 6 days of every week:

AVOID: Fruit, bread, rice, pasta, potato, milk, chocolates & sweets.

• This will put your body through a detox and you will experience extreme cravings for sugar but your body needs to break a habit of eating sugars, just like giving up smoking or drinking or caffeine.

• On the 7^th day, you can have ONE cheat meal where you can eat whatever you like. It is best to make this the evening meal otherwise if you have it earlier you will experience cravings for the rest of the day.

• Drink at least 2 litres of water a day. Whenever you are hungry or have a craving try eating a red pepper or celery with WholeEarth Peanut butter, or some cashew nuts or some raw carrots.

• Most meals should consist of protein (meat or fish), greens (broccoli/beans/spinach) and other veg. Salsa is useful to give flavour.

• Tinned sardines in olive oils are very healthy for snacking.

• Cook more meat/fish than you need in the evenings. The leftovers can be used as snacks during the next days.

• After 3 weeks, you can reintroduce apples/pears/grapes but no more than 1 apple/pear a day.

• Once you have gotten used to the diet your body will crave sugar/carbohydrate less and you will feel the true benefits.

EXERCISE

• If you combine diet with a good exercise programme your surgery will look so much better. If you lack motivation at the gym, invest in a personal trainer 3 times a week. They should focus on fat burning protocols and also core strength building rather than muscle bulking.

• Once you are in a good exercise and diet programme you can have a cheat day every 4-5^th day, but not for 3 months after starting the diet.

Male Breast Reduction with Vaser Liposelection

Gynaecomastia is a medical term which describes oversized breasts in men. It is an extremely common condition, with some studies reporting it to be present in up to 60% of men. It is thought that the incidence is increasing due to changes in environmental conditions in our diet over the last few decades.

There are two forms of gynaecomastia.

True gynaecomastia

This is when a breast gland has developed in response to a hormonal sensitivity, usually around puberty. It is often associated with extra fatty tissue in the breast which further increases the size of the breast. True gynecomastia may be diagnosed clinically during your examination with your surgeon or via an ultrasound scan. It needs to be treated surgically with excision of the gland. Liposuction or VASER may be used in addition, to remove any excess fat. A half circle incision is necessary around the outline of your nipple and the gland is removed through this. The procedure can be carried out ‘twilight’ or general anaesthesia, depending on the extent of the planned surgery. It can usually be performed as a day case, with no overnight stay required.

Pseudogynaecomastia

This is much more common that true gynaecomastia and this is when the enlarged breast is due to excess fatty tissue. The tissue is usually a mixture of fatty and breast tissue but hardly any breast gland. Excessive diets, alcohol and hormonal sensitivity are the most frequent causes. Pseudogynaecomastia can be treated very successfully with VASER Liposelection, usually through a couple of 4 mm incisions in your nipple zone, under ‘twilight’ anaesthesia for a completely pain free experience. Results last for many years and are often permanent as long as a healthy lifestyle is maintained.

What type of results can I expect from my gynaecomastia surgery?

At Riverbanks Clinic, we strive to achieve results which meet your expectations. During your consultation, it will have been explained to you that you should only proceed with VASER surgery if you will be a happy with 50% reduction in the pinch thickness of your breasts. You will have been shown several examples of previous Riverbanks patients  with varying degrees of success. During the surgery, the pinch thickness will be about 1 cm, however during your recovery, your skin layers will partially reform and the tissue thickness will increase slightly. We have a 90% satisfaction rate with our treatments and many men’s lives have been transformed following treatments. It is quite common for our patients to wish they had done this years ago.

What sort of recovery can I expect?

You will feel like you have done too many bench presses, or that you had been punched in the chest. In other words, your chest will feel tight and sore. It is not usually painful enough to require more than 1-2 days off work, but each individual’s response varies. After 2 days, you can take off your compression vest and remove your padding. Your scars should have healed up and you can have a quick cool shower. Continue with your antibiotics and wear your compression garment for 3 weeks 24/7 and then 3 weeks 12 hours a day. This is important to minimise any bleeding, inflammation and swellings and to maximise skin retraction.  You will be reviewed a week after surgery, when any stitches will be removed if they have been necessary. Occasionally at 6 weeks there will be some swelling present. This will usually resolve over the next 6 and will be reviewed at tour 6 month check-up with your surgeon.

When can I return to exercise?

Wait for 2 weeks after surgery before starting exercise. Be mindful that your chest may be tight with stretching movements. Because the surgery does not involve treating any muscles, your recovery will be relatively quick.

What can go wrong?

It is unusual for something to go wrong during VASER liposelection surgery. Common symptoms following surgery include pain and numbness and changes in nipple sensitivity. Increased or decreased sensitivity can last up to 6 months and in rare cases can be permanent. Chronic pain is not common.

What happens if I am not happy with my surgery?

In the unlikely event that you are not happy with your surgery, please discuss this with your surgeon. If there is no visible improvement or less than a 50% reduction in pinch thickness in either of your breasts, then your surgery will be revised at no extra cost to you. If there is a 50% reduction in pinch thickness but you would like further improvement, then further surgery may be booked at full price, if your surgeon feels that further surgery is indicated.

How much will my surgery cost?
